Description
After-hours assessment of specimens outside of one’s subspecialty training and practice can be daunting. At the faculty’s institution, STAT skin biopsies are not infrequent; these cases require prompt review in order to answer clinical questions, often influencing treatment decisions. This requires competency in triaging, working up, and answering the urgent clinical questions over the weekend to guide immediate management. The activity reviews the most common urgent/emergent dermatologic entities, the workup for each, and the treatment ramifications of the different diagnoses. The histologic features, including overlapping (and nonspecific) features, are covered to clarify the limitations as a pathologist. Finally, a case-based presentation demonstrates how to apply this knowledge and reviews essential verbiage to include in the preliminary report.

Objectives
After completing this activity, you should be able to:

  1. Evaluate dermatologic urgencies/emergencies that need assessment after hours.
  2. Identify common urgent/emergent dermatologic entities in the clinical differential and evaluate the pathologic workup.
  3. Identify the treatment ramifications of the commonly encountered dermatologic entities biopsied after hours.
  4. Recognize the histologic features of the most common dermatologic entities submitted for STAT assessment.
  5. Create a preliminary report of STAT dermatologic biopsies, incorporating the clinical history, to provide the necessary information to the treating clinician.
