Description
Upper and lower endoscopies are commonly performed to evaluate the gastrointestinal (GI) tract in patients with GI symptoms, such as abdominal pain and diarrhea. In patients with GI infections, biopsies are often intensely inflamed or ulcerated. However, some GI infections produce surprisingly little inflammation and can even show nearly normal histologic features. A high level of suspicion is necessary, particularly in specific patient populations, to recognize these infections and order the appropriate ancillary tests to arrive at the correct diagnoses. This activity discusses several GI infections and their risk factors, with an emphasis on those infections that are easily overlooked due to subtle histologic findings, and reviews ancillary techniques to confirm the diagnoses, as appropriate.

Objectives
After completing this activity, you should be able to:

  1. Develop a practical approach on how to work up infections in GI biopsies.
  2. Recognize GI infections that produce minimal or no inflammatory changes in GI biopsies.
  3. Identify the importance that patient population plays in the search for specific infections in the GI tract.
