Description
Discrepancies in serologic RhD testing can present diagnostic and therapeutic conundrums for clinicians. Discover which RhD variants can place patients at risk for developing anti-D upon exposure to the conventional D antigen through pregnancy or transfusion. Learn how molecular testing can be used to identify these at-risk patients.

Objectives
After completing this activity, you should be able to:

  1. Describe the algorithmic approach to a pregnant patient with an indeterminate RhD type by serology.
  2. Differentiate between transfusion management of patients with weak D variants compared to patients with partial D variants.
  3. Explain why an individual can have discrepant RhD testing as a patient versus as a blood donor.
  4. Identify the specimen requirements, utility, and limitations of molecular testing for RhD variants.
