Intended Audience
This clinical pathology activity is intended for pathologists.
Description
Hyperbilirubinemia occurs because of hemolysis, impaired conjugation, hepatocellular disease, or biliary obstruction. Distinguishing these conditions is aided through the measurements of total and conjugated bilirubin, urobilinogen, and hepatobiliary enzymes. These common tests provide an important window into hepatobiliary function that is necessary for the diagnosis and management of hepatobiliary disease. This activity will review proper interpretation of total bilirubin, conjugated bilirubin, urobilinogen, and the hepatobiliary enzymes that will assist the pathologist whether supervising a chemistry laboratory or serving as a surgical pathologist.
Objectives
After completing this activity, you should be able to:
- Describe the normal physiology of bilirubin and urobilinogen (eg, production and excretion).
- Compare and contrast the measurement of bilirubin and its fractions using the diazo reaction versus the dry-slide spectrophotometric measurement of unconjugated bilirubin (BU) and conjugated bilirubin (BC).
- Interpret plasma bilirubin and urine urobilinogen measurements in differentiating conjugated versus unconjugated forms of hyperbilirubinemia.
- Distinguish hepatocellular disease versus biliary tract obstruction through the measurement of the hepatobiliary enzymes ALT, AST, and ALP.
