Description
While most safeguards related to blood transfusion are proactive and related to preventing transfusion of unsuitable/potentially unsafe products, additional safeguards must exist for the scenario where an unsuitable product was collected, shipped to a transfusion service, and potentially unknowingly transfused. This safeguard is called a “lookback” and refers to the process of investigating and identifying prior donations from the same donor that might have been collected during the “window period.” Pathologists covering a transfusion medicine service must be aware of these processes in the interest of patient safety and compliance with federal regulations and accreditation standards.
Objectives
After completing this activity, you should be able to:
- Describe safeguards implemented to prevent relevant transfusion transmissible illness.
- Describe the process for “lookback” investigations.
- Evaluate and apply a framework for practicing sound, ethical decision-making in blood product lookbacks.
